Documents Required

01. Print out of duly-filled and signed visa application form.

02. Two photographs

03. Passport valid for three months beyond your departure from Spain

04. A copy of data page of your passport

05. Details of roundtrip flight reservation

06. Travel Medical Insurance

07. Proof of accommodation in the hospital/hotel, letter of invitation, or rent contract.

08. Proof of civil status such as marriage certificate, birth certificate of children, death certificate of spouse, if applicable

09. Proof of paid fee

10. Proof of funds to cover the travel and medical expenses during the entire stay. This includes a bank statement, letter of sponsorship, proof of advance payment, and a verbal note from the Ministry of Health of the applicant's home country declaring the agreement of the government to cover the cost of your treatment.

1.3. Pay slips for the last 6 months

1.4. Income Tax Return

02. If Self-Employed:

2.1. A copy of your business license,

2.2. Company bank statement of the latest 6 months

2.3. Income Tax Return (ITR)

03. If a Student:

3.1. Proof of University enrolment

3.2. No Objection Letter from University

04. If Retired:

4.1. Pension statements of the latest 6 months

Documents to prove the need of Medical Treatment:

01. A letter from a doctor / clinic / hospital in the home country of the candidate, that he or she needs medical treatment in Spain.

02. Official confirmation of the treatment/appointment by the receiving medical institution (hospital/clinic) in Spain.

03. Proof of advance payment of the treatment / financial arrangements with the medical doctor.

04. A guarantee letter from the applicant to assure the patients capacity to cope with the extra medical costs (when expecting unforeseen costs of medical treatment to happen)

05. Confirmed Accommodation in the Medical Clinic

Additional Documents for Minors:

01. Birth Certificate. Provide this certificate in original, accompanied with a copy.

02. Letter of Consent (whichever applicable)

2.1. From the other parent.

2.2. From both parents.

2.3. Death certificate of parent.

Step by Step Procedure for Visa Application

01. Fill out the visa application form and submit the application for (short or long period stay)

02. Collect all the supporting documents applicable in your case

03. Make an appointment in the embassy

04. Attend the interview with the documents and pay the visa fee

05. Wait for the visa.

It takes approximately 15 days to process the medical visa to Spain. It allows you to stay for 90 days within a period of six months. If you need to stay for a long period of time, you need to apply for a Long-stay National visa for medical treatment. If you wish to extend your visa validity, you will need to prove that you are unable to travel out of Spain due to bad health conditions or you need to continue the treatment in Spain.

You must consider the following points while submitting the photographs in the supporting documents:

01. Size must be 35x45mm

02. Photograph must be colored

03. Your head should cover 70-80% of the photo

04. Background should be light grey with no patterns

06. Face expressions should be neutral with the mouth closed and no smile.

07. Avoid wearing clothes matching the background.
